Sports Medicine Institute of Indiana’s Indianapolis facility is located on the newly renovated campus of Community Hospital North, a state-of-the-art facility accommodating all of the ancillary needs of our patients. Sports Medicine Institute of Indiana is conveniently located on the fifth floor of the Indiana Surgery Center Building. Sports Medicine Institute of Indiana’s second facility is located in Muncie, Indiana and is staffed by Dr. Combs.

Insurance and Payment
Thank you for selecting Sports Medicine Institute of Indiana’s qualified surgeons for your orthopedic health care needs. Our physicians are committed to providing quality, appropriate medical care to our patients. The Board-Certified Orthopedic Surgeons at Sports Medicine Institute of Indiana participate in most Health Plans and Workers’ Compensation.
The physicians at Sports Medicine Institute recommend treatment based upon the patients medical needs. Please be aware some services may be “non-covered” services under the provisions of your medical insurance. You are still responsible for the bill. Payment for out-of-network charges is also the patient’s responsibility. It is the patient’s right to refuse treatment.
All copays will be collected at the time of service. The patient is responsible for all coinsurance, deductibles and copays. We will send you a statement that summarizes what service(s) were received, the date(s) of services, charges and the amount your insurance carrier paid. Payment is due within ten days of receipt of your statement. For your convenience we accept cash, check, MasterCard, Visa and Discover. If you wish to discuss payment plan, please call our business office.
