Established patients of Sports Medicine Institute of Indiana can conveniently request a prescription and refills Monday through Friday between the hours of 8 AM and 5 PM. Our Interactive Prescription Request Form makes it easy.  Simply complete the form and click submit. You can also make your request by calling our office at (317) 841-8326 or having your pharmacy fax the request to us at (317) 841-9195. Please allow 24 hours for refill requests.

No refills are issued during the evenings, weekends, or holidays, as the doctors do not have access to your medical records during those times.  Please anticipate your refill needs to avoid an interruption in your medication therapy.
